For Anticoagulation in the presence of HIT (Heparin-Induced Thrombocytopenia)

A. Use on CardioPulmonary Bypass , CPB (pdf file from Univ. of Michigan)

B. For Non-Cardiac Surgery (Chase Boyd March 2012)

  • Initial Bolus by syringe 0.5 mg/kg
  • to maintain desired ACT (usually 1.5-3x baseline)
  • Infusion: 1 mg/kg/hr (titrated to ACT)
  • If on HemoDialysis: 0.25 mg/kg/hr (titrated to ACT)
  • Continue infusion until 15 min. prior to anticipated end
  • Infusion may need to be decreased in renal insufficiency or hypothermia, monitor ACT closely
